Objective To summarize the best evidence of exercise rehabilitation in lower extremity arteriosclerosis obliterans(LEASO)patients complicated with type 2 diabetes mellitus(T2DM)after endovascular intervention.Methods The research questions were formulated using the PIPOST framework,and a systematic search was conducted using a top-down approach in accordance with the"6S"evidence-based pyramid model,the literature about exercise rehabilitation in LEASO patients complicated with T2DM after endovascular intervention was searched in guidelines and evidence-based resource platforms(BMJ Best Practice,UpToDate,National Guideline Clearinghouse,Scottish Intercollegiate Guidelines Network,Guidelines International Network,National Institute for Health and Care Excellence website,Registered Nurses'Association of Ontario website,Australian Joanna Briggs Institute Based Healthcare Research Center website),professional societies and institutions(American Heart Association,American Diabetes Association,Agency for Healthcare Research and Quality,medlive,Chinese Medical Association)websites,and Chinese/English academic databases(Cochrane Library,PubMed,Embase,Web of Science,EBSCO database,Ovid database,CBM,CNKI,Wanfang Data,VIP),the search time spanned from the inception of each database to December 31,2025.Two researchers independently evaluated the quality of the included literature,and extracted and summarized the evidence.Results A total of 23 articles were included,including 9 randomized controlled trials,5 guidelines,4 systematic reviews,3 evidence summaries,1 clinical decision,and 1 expert consensus.Finally,19 best-practice evidence of exercise rehabilitation in LEASO patients complicated with T2DM after endovascular intervention across 7 key domains were developed:pre-exercise assessment and baseline measurement,selection of content and form of exercise prescription,exercise dosage parameters and progression principles,implementation of professional supervision and multidisciplinary team collaboration,exercise monitoring and safety management,medication-related exercise considerations,and outcome evaluation.Conclusion This study has synthesized the best evidence of exercise rehabilitation in LEASO patients complicated with T2DM after endovascular intervention,with 19 pieces of evidence in 7 aspects,including pre-exercise assessment and baseline measurement,selection of content and form of exercise prescription,exercise dosage parameters and progression principles,implementation of professional supervision and multidisciplinary team collaboration,exercise monitoring and safety management,medication-related exercise considerations,and outcome evaluation.
